Describe what you want to find out in one sentence. You'll get a balanced set of survey questions — ratings, choices and open questions — you can copy anywhere or turn into a form.
Say who you're asking and what you'll do with the answers. “Feedback from parents after a school trip, to plan next year's” gets far sharper questions than “school trip survey”.
Then edit. Delete anything you won't act on — every extra question costs you answers — and keep the easy questions first so people who stop early still tell you something.
Ask one thing at a time. “Was the event fun and well organised?” can't be answered honestly by someone who enjoyed it but found it chaotic.
Don't lead. “How much did you love the new menu?” nudges people towards a nice answer. “How would you rate the new menu?” doesn't.
Ask about what happened, not what might. “How many times did you order last month?” is more honest than “How often would you order?”.
